Financial

• Please understand that as the patient, you are ultimately responsible for the payment of your account. Your responsibility at the time of the office visit is:
  • Co-Payments
    
    
  • Any deductible or co-insurance for office visits and office procedures
    
    
  • Any deductible or co-insurance for scheduled surgeries
    
    
• We accept cash, checks, VISA, Mastercard, American Express, Discover and any certified funds

• For hospital/surgery center billing questions, please contact the facility. You may also receive billing from anesthesiology, pathology and radiology. These are all separate billing entities.

Cancellations

If an appointment or a surgery needs to be cancelled – please do so at least 24 hours in advance of the scheduled appointment and 3 days (72 hours) for surgery. If the patient fails to cancel at least 24 hours in advance there will be a fee of $25.00 for office appointment. If the patient fails to cancel at least 3 days (72 hours) there will be a fee of $100.00 for surgery.

Tests/Procedures

We send all specimens to ***CLINICAL PATHOLOGY*** unless otherwise notified.

It is the patient’s responsibility to make sure any diagnostic studies or lab work requested by our doctors is scheduled at their insured’s contracted facility
