We want to make sure that we have everything we need to ensure that you receive the best service possible. Therefore, please hand carry to your appointment any films needed to determine your diagnosis.

Please inform your referring physician's office of your appointment, so that any pertinent medical records can be faxed to us in advance of your appointment. Our fax number is (512) 467-8809.

Make sure you bring your current insurance card to your appointment, along with your referral number if necessary. Co-payments are due at the time of service.
